By requesting make-up services from READY SET GLAM you agree to the following:
1. I give my permission to receive a make-up service from READY SET GLAM and the provider performing the service.
2. I release READY SET GLAM and the provider performing my service of any liability associated with any injuring and/or current and future conditions resulting from the make-up allocation, procedures, or products.

2. I understand that I, or the make-up artist, may terminate the make-up session at any time.
3. I acknowledge that if I am allergic to one or more ingredients in the products used, I may experience allergic reactions. That my skin might experience temporary irritation, tightness, redness, or slight swelling which usually dissipates within a short period of time depending on skin sensitivity.
4. I acknowledge that this treatment is strictly an elective cosmetic procedure and no medical claims have been expressed or implied.
5. I consent (to the best of my knowledge) that the answers I have given are correct and that I have not withheld any information that may be relevant to my treatment.
